Fillers vs. Botox®: What’s Better for Lips, Cheeks & Wrinkles?

BOTOX®, a neuromodulator, works by temporarily blocking the chemical signals from the nerves that tell these specific muscles to contract. By relaxing the underlying muscle, the skin above it smooths out, and the wrinkles soften or disappear entirely.

Dermal fillers, gel-like substances, can be injected beneath the skin to provide an array of benefits, including restoring lost volume, smoothing lines, and enhancing facial contours. Common options include Juvéderm® and Restylane® (injections with naturally-occurring hyaluronic acid), Radiesse®, and fat injections. 

Dr. Zapiach says dermal filler results often appear right away and can last for years, depending on the particular procedure. He adds that bruising and swelling can be expected for all dermal filling techniques; however, most patients can immediately return to their daily activities.

  • Lips: Lip fillers are almost always the better choice for lip enhancement. They add subtle or significant volume, define the lip border, smooth smoker’s lines, and create symmetry. BOTOX® can be used for a “lip flip,” but it does not add volume — only relaxation of the upper lip.
  • Cheeks: Fillers are ideal for rejuvenating the cheeks. They can lift the midface, restore plumpness, and create a more youthful contour.
  • Upper-Face Wrinkles: If your wrinkles appear on your forehead or around your eyes when you make a facial expression, choose BOTOX®.
